Home care focuses on non-medical support such as personal care, companionship, meal preparation, housekeeping, and transportation. Home healthcare includes clinical services provided by regulated healthcare professionals, such as nursing care, medication management, wound care, and health assessments.
Many people benefit from home care if they are finding everyday tasks more difficult, recovering from surgery or illness, living with a chronic health condition, experiencing memory changes, or if family caregivers need additional support. We offer a complimentary consultation to discuss your needs and determine whether home care is the right fit.

When a client has the capacity to make their own decisions and provides informed consent, we can collaborate with physicians and other healthcare providers to help coordinate your care.
Respite care provides temporary relief for family caregivers by ensuring their loved one continues to receive safe, compassionate care while they take time to rest, attend appointments, or manage other responsibilities.
We offer post-hospital support including nursing care, medication reminders, personal care, mobility assistance, meal preparation, companionship, and coordination with other healthcare providers to help ensure a safe recovery at home.
Yes. We can accompany clients to medical appointments, assist with transportation, provide support during the visit, take notes if requested, and help communicate important information back to family members with the client’s consent.
Absolutely. Our caregivers can assist with grocery shopping, meal planning, meal preparation, and light household tasks to help clients maintain a healthy and independent lifestyle.
Yes. We believe mental and emotional well-being are essential parts of overall health. Our Registered Nurses (RNs) and Registered Social Workers (MSWs) provide compassionate, person-centred support to help clients navigate emotional challenges while living safely and comfortably at home.

1. Initial Contact – We begin with a conversation to learn about your needs, answer your questions, and determine how we can best support you or your loved one.
2. Comprehensive Assessment – A Care Coordinator or Registered Nurse meets with you to discuss your health, daily routines, goals, preferences, and home environment. Together, we develop a personalized care plan that reflects what matters most to you.
3. Care & Services Begins – We carefully match you with a caregiver or healthcare professional based on your needs, personality, and schedule. Services may include nursing care, personal support, companionship, respite care, transportation, or other home care services.
4. Ongoing Communication – Your care plan is regularly reviewed and adjusted as your needs change. Our Care Coordinator and Registered Nurse Supervisor remain available to answer questions, address concerns, and ensure you continue receiving high-quality, person-centred care.
5. Transition or Discharge – When your goals have been achieved or your care needs change, we work with you, your family, and other healthcare providers to ensure a smooth transition. Whether services are reduced, ended, or transferred to another level of care, we strive to make the process seamless while continuing to support your well-being.
Your care plan is created in collaboration with you and, with your consent, your family or substitute decision-maker. It outlines the services you will receive, your goals, your preferences, visit schedules, and any important health or safety considerations. Because every person is unique, your care plan is personalized and flexible.
As your care continues, your caregiver will provide the agreed-upon services while encouraging your independence and respecting your choices. Your Care Coordinator and Registered Nurse Supervisor remain available to answer questions, monitor your progress, and adjust your care plan as your needs change.

Depending on your needs, your care may be provided or coordinated by a variety of qualified healthcare professionals and support staff, including:
- Registered Nurse (RN) – Comprehensive nursing assessments, clinical care, medication management, wound care, chronic disease management, palliative care, care coordination, and clinical supervision.
- Registered Practical Nurse (RPN) – Nursing care, medication administration, health monitoring, chronic disease support, wound care, and client education.
- Personal Support Worker (PSW) – Assistance with personal care, mobility, companionship, meal preparation, light housekeeping, respite care, and activities of daily living.
- Registered Social Worker (MSW/RSW) – Counselling, mental health support, caregiver support, grief and bereavement counselling, care planning, advocacy, and community resource navigation.
- Care Coordinator – Coordinates services, develops and updates care plans, schedules visits, communicates with families, and ensures continuity of care.
- Companion Care Provider – Social engagement, conversation, recreational activities, accompaniment to appointments, safety checks, and emotional support.
- Home Support Worker – Assistance with housekeeping, laundry, meal preparation, grocery shopping, errands, and maintaining a safe home environment.
- Palliative Care Provider – Comfort-focused care, symptom support, emotional support, and collaboration with healthcare providers to enhance quality of life.
- Dementia Care Specialist – Person-centred support for individuals living with dementia, including cognitive engagement, behavioural support, routine building, and caregiver education.
- Foot Care Nurse – Routine foot assessments, nail care, diabetic foot care, and education to promote foot health and prevent complications.
- Community Health Navigator – Helps clients and families access healthcare services, funding programs, community resources, and specialist referrals.

Yes. All of our care services are overseen by a Registered Nurse (RN) who provides clinical leadership, guidance, and ongoing supervision. The RN helps develop and review care plans, monitors changes in clients’ needs, and supports our caregivers to ensure care is safe, consistent, and tailored to each individual. This collaborative approach promotes high-quality, person-centered care and provides added confidence for clients and their families.
Yes. Our team receives ongoing training in dementia care, communication strategies, person-centered approaches, and techniques that promote safety, dignity, and meaningful engagement.
Yes. When a client has the capacity to make their own decisions and provides informed consent, we encourage family members and loved ones to participate in developing and reviewing the care plan. This collaborative approach helps ensure care reflects the client’s wishes, values, and goals while fostering open communication and ongoing support.
It can be difficult when you have concerns about a loved one’s choices. If your loved one has the capacity to make their own healthcare decisions, they have the legal right to make choices about their care—even if others disagree. Our role is to respect their autonomy while ensuring they have the information and support needed to make informed decisions. We encourage open, respectful conversations that include the client and, with their consent, family members whenever possible. If there are concerns about a person’s decision-making capacity or safety, our team can help guide families through the appropriate assessment and next steps.
Changes such as missed medications, falls, poor nutrition, memory concerns, difficulty managing household tasks, or increased isolation may indicate additional support is needed. We can complete an assessment and recommend services that help promote safety and independence.
Before any care begins, we will explain the recommended services, their purpose, any potential risks or benefits, and available alternatives. You have the right to ask questions, make informed decisions, and accept or decline services at any time. Your consent is always respected and may be withdrawn whenever you choose.
Capacity is a person’s ability to understand information relevant to a decision and appreciate the potential consequences of that decision. In Ontario, adults are presumed to have capacity unless there is reason to believe otherwise. If there are concerns about decision-making capacity, we follow Ontario legislation and involve the appropriate substitute decision-maker when required.

Absolutely. Our nurses are licensed and regulated by the appropriate professional college in Ontario and must meet established standards of practice and ethics. Personal Support Workers and other care providers receive ongoing training, supervision, and guidance from our Registered Nurse leadership team to ensure consistent, high-quality, person-centred care.
We regularly review care plans, communicate with clients and families, supervise our care team, and monitor changes in clients’ needs. Ongoing education, quality improvement initiatives, and adherence to provincial legislation and professional standards help ensure every client receives compassionate, safe, and reliable care.

Yes. We provide care seven days a week, including evenings, weekends, and holidays.
Yes. We ask for advance notice whenever possible. Our cancellation policy will be reviewed during intake.
Costs vary depending on the type of service, length of visits, and frequency of care. We’ll provide a clear quote before services begin.
Many extended health insurance plans reimburse home care services. Coverage varies, so we recommend checking with your insurance provider.
Privately arranged home care is generally not covered by OHIP. However, some publicly funded services may be available through Ontario Health atHome if you qualify.
