to help you remain safe and comfortable in your own home.
Every person’s care journey is unique, and so are their needs. While our services are organized into categories, care often overlaps. A client receiving nursing care may also benefit from personal support, companionship, or help
around the home, while someone receiving companion care may later require additional clinical or specialized services.
Nursing Services
Our Registered Nurses (RNs) and Registered Practical Nurses (RPNs) provide skilled clinical care tailored to your individual health needs. Services include:
- Health assessments – Evaluating your overall health and care needs.
- Medication administration – Safely giving prescribed medications.
- Medication management – Organizing and monitoring medications.
- Chronic disease management – Supporting ongoing health conditions.
- Diabetes management – Helping manage diabetes safely.
- Blood glucose monitoring – Checking and tracking blood sugar levels.
- Wound care – Cleaning, dressing, and monitoring wounds.
- Ostomy care – Caring for and managing ostomy needs.
- Catheter care – Monitoring and maintaining urinary catheters.
- Continence care – Support with bladder and bowel health.
- Health education – Teaching clients and families about health and wellness.
- Palliative care – Comfort-focused care for serious illness.
- End-of-life support – Compassionate care during the final stages of life.

We understand that reaching out for home care can feel like a big step. Whether you’re planning ahead, recovering from an illness, or looking for support for a loved one, our team is here to make the process simple, supportive, and personalized.
From your first phone call to your first home visit, we’ll guide you every step of the way and ensure you feel informed, comfortable, and confident in your care.
1. Initial Contact
Your first contact is an opportunity for the Care Coordinator to get to know you and understand how we can help.
You can use the Contact Form, Intake Form, or contact the office directly at 226-407-4079.
During this conversation, we will:
- Listen to your concerns and answer your questions.
- Learn about your current health, lifestyle, and support needs.
- Explain the home care services we offer.
- Discuss your goals, preferences, and expectations.
- Provide information about scheduling, pricing, and available care options.
- Arrange a complimentary in-home assessment if you’d like to move forward.
There is no obligation to begin services. Our goal is simply to provide information and help you determine the best next steps for you or your loved one.
2. Nursing Assessment
Your assessment is one of the most important steps in creating safe, personalized care.
A Registered Nurse or Care Coordinator will meet with you in your home to gain a complete understanding of your needs and how we can best support you.
During the assessment, we may discuss:
- Your health history and current medical conditions.
- Medications and treatment needs.
- Your daily routines and level of independence.
- Mobility, balance, and fall risks.
- Personal care needs.
- Nutrition and meal preferences.
- Memory, cognition, and communication.
- Emotional well-being and mental health.
- Your home environment and safety considerations.
- Family involvement and available supports.
- Cultural, spiritual, and personal preferences.
- Your goals for care and quality of life.
You’ll also have plenty of time to ask questions, discuss concerns, and learn more about our approach to care.
3. Care Plan
Following your assessment, our team develops a personalized care plan based on your unique health needs, goals, preferences, and daily routines. Whenever possible, we create the plan together with you and, with your consent, your family or substitute decision-maker.
Your care plan may include:
- Nursing services
- Personal support
- Companion care
- Home support
- Mental health support
- Specialized care services
- Visit schedules and frequency
- Health and safety considerations
- Goals for maintaining independence and well-being
Your care plan is a living document that is reviewed regularly and updated whenever your needs change to ensure you continue receiving the right level of care.
4. First Visit
Your first visit is designed to help you feel comfortable, welcomed, and supported.
Your caregiver will:
- Introduce themselves and explain their role.
- Review your care plan with you.
- Confirm your preferences and daily routine.
- Begin providing the agreed-upon services.
- Answer any questions you may have.
- Ensure you feel comfortable with the care being provided.
- Document important observations and communicate with the care team as needed.

4. Ongoing Support
